Where Psychiatry Practices Lose Revenue
Psychiatry revenue rarely disappears in one big denial. It leaks a few dollars at a time, on visits that were billed but billed short.
01
Psychotherapy Add-Ons Left Off Medication Visits
When a prescriber provides psychotherapy during a medication management visit, the visit can support two codes: an E/M and a psychotherapy add-on. If the therapy time isn't documented separately, or the biller doesn't know to look for it, the practice is paid for half the work.
02
E/M Levels Set Too Low
Psychiatric patients often carry multiple chronic conditions and prescription drug management, which supports moderate medical decision making. Practices that default to 99213 for every follow-up leave money on almost every visit.
03
Claims Sent to the Wrong Payer
Many health plans route behavioral health benefits to a separate behavioral health company. A psychiatry claim sent to the medical plan instead of the carve-out vendor comes back rejected, often weeks later.
04
Authorizations for Interventional Treatment
Spravato and TMS require prior authorization with documented failed medication trials and treatment history. A missing trial or expired approval turns an expensive course of treatment into an unpaid one.
05
Telepsychiatry Billed With the Wrong Rules
Place of service, telehealth modifiers, and audio-only rules vary by payer. One wrong combination can deny an entire month of virtual follow-ups.

Psychiatry Billing Rules That Matter in Nevada
01
Psychiatric nurse practitioners bill independently. Nevada grants APRNs full practice authority, so PMHNPs can be credentialed and bill under their own NPI. Nevada Medicaid enrolls physicians as Provider Type 20 and APRNs as Provider Type 24.
02
Nevada Medicaid managed care is statewide. Since January 2026, managed care covers every county. Psychiatry practices may bill Anthem, CareSource, Health Plan of Nevada, Molina, and SilverSummit, each with its own behavioral health authorization rules.
03
Medicare is processed by Noridian. Nevada's Medicare Part B claims go to Noridian Healthcare Solutions (Jurisdiction E), and Medicare Advantage plans add their own authorization requirements.
04
Demand outpaces supply. Behavioral health has the most severe workforce shortage of any specialty area in Nevada, according to the Nevada Health Authority's 2026 workforce assessment. Full schedules leave little staff time for denials and follow-up.

Psychiatry Coding Expertise That Captures the Whole Visit
The most common psychiatry billing mistake is billing a two-part visit as one. Here's how a medication management visit with psychotherapy should be coded.
One medication visit can bill as two codes
Medication visit · psychiatrist or PMHNP
E/M service
prescribing, symptoms, risk
Claim line 1: 99212–99215
Level from MDM, never therapy time
Psychotherapy
minutes noted separately
Claim line 2: add-on by minutes
9083316–37 min
9083638–52 min
9083853+ min
Both lines go on the same claim; the add-on is paid only alongside the E/M.
E/M plus psychotherapy add-on · one visit, two claim lines
The E/M level is chosen by medical decision making only: problems addressed, data reviewed, and risk, including prescription drug management. Psychotherapy time is documented separately and can't be counted toward the E/M level. The add-on code is chosen by those psychotherapy minutes.
Not sure your add-ons are being captured? Get a Free Psychiatry Billing Audit and we'll check a sample of your medication visits.
CPT 90791 and 90792 | Psychiatric Diagnostic Evaluation
90792 is for evaluations that include medical services, such as medication review and prescribing, and is billed by psychiatrists and PMHNPs. 90791 is the evaluation without medical services. We confirm the right code by provider and documented content, and check each payer's limits on repeat evaluations.
02
CPT 99202–99215 | Medication Management E/M
New and established patient visits leveled by medical decision making or total time (when no psychotherapy is billed). We audit your level distribution so complex psychiatric patients aren't routinely billed as 99213.
03
CPT 90833, 90836, 90838 | Psychotherapy Add-Ons
Billed only with an E/M on the same date, by the same prescriber, based on documented psychotherapy minutes. When the payer requires it, we apply modifier 25 or 59 per that payer's policy.
04
CPT 90785 | Interactive Complexity
An add-on for visits where communication is complicated, such as caregivers with conflicting goals, emotions that interfere with care, or a required report of a sentinel event. We apply it only when documentation supports it.
05
CPT 90839 and 90840 | Crisis Psychotherapy
90839 covers the first 30–74 minutes of crisis care and 90840 each added 30 minutes. Time and the crisis nature of the visit must be documented.
06
CPT 99417 and G2212 | Prolonged Services
For long E/M visits billed by time, we use 99417 for commercial plans and G2212 for Medicare, based on each payer's time thresholds.
07
CPT 99484, 99492–99494, 99446–99452 | Integrated and Consultative Care
General behavioral health integration, the Collaborative Care Model, and interprofessional consults with primary care. We set up the right billing practitioner and monthly time tracking. primary care billing services

Psychiatry billing
How Much Does a Psychiatry Billing Company Charge?
Psychiatry billing services are usually priced as a percentage of monthly collections. Behavioral health billing commonly runs 4–9%, and outpatient psychiatry practices usually fall in the lower half of that range. Practices with heavy Spravato or TMS authorization work, or large old A/R, fall higher. There are no setup fees, the agreement is month-to-month with 30-day notice, and you get a fixed quote after your free audit.
Worried About Switching? Here's How We Remove the Risk
01
"Our cash flow will drop." We work old A/R in parallel while new claims go out.
02
"Our Spravato and TMS auths will lapse." Every active authorization is loaded before go-live.
03
"We'll lose visibility." You see every claim in your own EHR, plus a monthly review call.
04
"Our patients' privacy is at risk." We sign a BAA and limit access by role.
05
"We'll be locked in." You won't. The agreement is month-to-month with 30-day notice.

Get Paid for Every Part of Every Visit
We help Nevada psychiatric practices capture add-ons, level visits correctly, secure Spravato and TMS authorizations, and clear old A/R.
Psychiatry billing services cover benefits verification, coding, prior authorization, claim submission, payment posting, denial appeals, A/R follow-up, credentialing, and patient billing for psychiatrists and psychiatric nurse practitioners.
Most charge a percentage of monthly collections. Behavioral health billing commonly runs 4–9%, and outpatient psychiatry practices usually fall in the lower half. We give a fixed quote after a free audit.
Yes. When a prescriber provides both, bill the E/M (leveled by medical decision making) plus a psychotherapy add-on: 90833 for 16–37 minutes, 90836 for 38–52, or 90838 for 53 or more. Therapy time can't count toward the E/M level.
90792 is a psychiatric diagnostic evaluation that includes medical services, such as medication review, and is billed by prescribers. 90791 is the evaluation without medical services.
For Medicare, Spravato visits use G2082 (up to 56 mg) or G2083 (over 56 mg), which include the two-hour observation. Commercial plans vary, often using E/M or observation codes plus a drug code. The site must be REMS-certified, and most plans require prior authorization.
TMS uses 90867 for the initial session with motor threshold mapping, 90868 for each later session, and 90869 for re-mapping. Plans usually require prior authorization showing failed medication trials.
Yes. Nevada grants nurse practitioners full practice authority, so PMHNPs can enroll and bill independently with Medicare, Nevada Medicaid, and commercial plans.
We apply each payer's place of service (02 or 10), telehealth modifier (95 or GT), and audio-only rules, and track Medicare's telemental health requirements.
Yes: Anthem, CareSource, Health Plan of Nevada, Molina, and SilverSummit, plus fee-for-service Nevada Medicaid.
